    About this opportunity:The British Red Cross Assisted Discharge Service supports Fife patients during the transition from hospital to home. We will initially meet with the person using the service the day before to allow a better understanding to meet needs required and to make the service user more comfortable and confident leaving the hospital. On the day of discharge the service user will be collected/dropped off via hospital transport from agreed hospital and supported to settle back in their own home. Support time can vary depending on needs of service user. A follow up call 24/48 hours later again depending on the needs of the service user

    Training provided:A 3 day course of Foundation Training Programme.
    Out of pocket expenses paid?
    Yes - 45p per mile petrol.

    Experience required:Driving Licence, Caring, Compassionate, Empathetic

    Travel DetailsBRC can take referrals for NHS Fife when required. Travel to hospital based in Fife to designated destination. Support from Service Co-coordinator based in Victoria Hospital
