As Fife Health and Social Care Partnership moves towards finalising its Transitions Protocol, it is vital that the voices of those who support young people to transition from children’s services into adult’s services are at the heart of the process.
Through the Transitions Protocol, they are improving the way we support young people with additional or complex needs as they transition into adult’s services. These improvements are designed to make planning for transitions earlier, clearer and less stressful for young people and their families/carers.

They are also asking for your support in sharing the surveys below with the Young People and their Families/Carers that you have supported to transition into adult services.
