    The Self Management Fund is now open for applications. 

    The fund will continue to focus on areas around proactive support, reducing barriers to healthcare, and supporting people on waiting lists. 

    They will distribute Scottish Government funds to third sector and community organisations in Scotland to support new activities that will encourage new approaches or add to their learning around self management. 

    Two levels of grant are available:

    • Small grants of up to £15,000 for up to one year
    • Large grants of up to £120,000 for up to three years

    In 2025 the Self Management Fund aims to support projects that put people and communities at the centre. Applications must demonstrate their project supports, guides and enables self management in one of the following ways:

    • Supporting people at an early stage in their journey with a long term condition, disability or unpaid caring responsibility, to develop their ability to self manage
    • Reducing barriers to accessing health and social care support and services, to enable self management for people living with long term conditions, disabilities and/or unpaid carers
    • Developing the self management capacity of people living with long term conditions, and/or their unpaid carers, while they wait for a specialist health or social care intervention

    While it is possible for a project to meet more than one priority, projects are no more likely to be successful if they meet multiple priorities. Therefore, they are asking applicants to focus their applications on the priority they feel their project demonstrates the strongest fit with. They hope that this change will support applicants to tell a clearer story of the change they hope to make.

    Who can apply

    Applications are open to third sector and community organisations working with people to support self management.

    An online application form is available. You will need to create an account if you do not already have one. You will be able to save your application and come back to it.
