This event is being organised by Fife Voluntary Action
It is too late to book a place for this particular session, sorry about that.At this meeting, we invite Fife Health and Social Care Partnership's Head of Strategic Planning, Performance Commissioning, Fiona McKay along with the Participation and Engagement Team to engage with you on the Home First vision which aims to ensure that everyone in Fife is able to live longer healthier lives at home, or in a homely setting. You will be asked by the team your thoughts on the vision, what might be some of the challenges and what has been missed.
Your thoughts and insight will help inform the development of a Home First Strategy for Fife which will limit admission to hospital, establish a timely discharge from hospital, and support the delivery of person-centered transfers of care in a responsive integrated system.
We are running this Forum in person at our office in Glenrothes - Caledonia House, Pentland Park, Saltire Centre, Glenrothes, KY6 2AL - as the activities we are running are better suited for face-to-face engagement rather than online, so we do strongly encourage you to attend in-person if you can.
If you can't attend in person, we do have an online option to join the event via MS Teams. Please e-mail Eloise (Policy and Communications Officer, FVA) at [email protected] if you plan to attend online. This is so we can establish the numbers for possible breakout rooms.
