Fife H&SCP brings Holistic Needs Assessment (HNA) approach to Levenmouth
Created: 25/08/2021, Updated: 22/09/2021Fife Health & Social Care Partnership is delighted to bring the Holistic Needs Assessment (HNA) approach to Levenmouth, as part of a test of change project funded by Macmillan, working in partnership with Fife H&SCP (NHS Fife and Fife Council), Fife Forum, Fife Carers Centre and Citizens Advice and Rights Fife.
The project offers a central point of access (CPA) for health and social care professionals within the Levenmouth area, to refer people living with COPD and Heart Failure, who may benefit from receiving a Holistic Needs Assessment. Check out this newsletter on the project!
The Partnership will be using the Macmillan’s electronic Holistic Needs Assessment (eHNA) tool, currently being utilised by Fife’s Improving the Cancer Journey Service, which facilitates a good conversation between the individual and key worker, resulting in a care plan identifying what is important to the person.
The project is now live and will run until the end of January 2022, exploring the benefits of this approach for the person living with a long-term condition and for all the health & social care professionals supporting that person to live a more independent and healthier life.
