Patient Transport - Continuing Care Service Service

Last updated: 25/08/2026 (more than 1 month ago)Continuing Care Service provides non-emergency transportation for the elderly, disabled and vulnerable. The current service is for patients who are unable to drive themselves and require to present themselves for appointments at health service providers. They are available Monday to Friday for all local and longer distance transportation. Weekend appointments can be undertaken by special arrangement.
Usually up to two days notice is required to enable them to schedule volunteer drivers, although they will endeavour to accommodate appointments at shorter notice.
