They support individuals to maintain their independence, dignity, wellbeing and quality of life while enabling them to remain safely within their own communities. Their services are tailored to each individual’s assessed needs, personal outcomes, preferences and choices. Support may include personal care, assistance with daily living activities, medication support where appropriate, community participation and other agreed care and support needs. They work in partnership with individuals, their families and carers, social work, health and social care professionals and other relevant organisations to ensure that support is safe, responsive, respectful and outcome-focused. They are committed to the principles of Self-directed Support (SDS), including supporting individuals who choose to manage their care through appropriate SDS arrangements. They aim to provide reliable, compassionate and flexible care that promotes choice, independence, inclusion and meaningful participation in community life.
